The Notebooking Nook - has a state study notebooking pages for each state. I only use a few pages for my 3rd grader (basic state facts, where's Utah, etc.) This costs a few dollars but really nice pages.
Enchanted Learning - I print out their state maps which asks 10 questions which all answers could be found on the map. Also, I print out the state flags which allows the students to color the flags and gives a little information about the state and flag followed by a few questions. Free.
Then I made up my own sheet for each state that wasn't covered in the other pages which includes:
state abbreviation
largest city
major industry
major resources
major rivers
major lakes
wild life
highest/lowest point
origin of state name
what is the state famous for
name something unique about the state
Then on separate piece of paper, list what you would do and where you would go in the state and why.
